After completing registration, please review the forms below and complete the ones that apply to your family.

Additional Toddler and Preschool Forms:

Child Enrollment and Health Information for Toddlers and Preschool

Medical Statement for Toddlers and Preschool to be Signed by Doctor

Medical forms are only required for the Toddler and Preschool divisions.
For campers in the Boys and Girls Elementary divisions and above, the permission and acknowledgment form completed during registration gives us access to the necessary medical information, so no additional medical forms are required.

For Campers Requiring Medical Intervention or Additional Medication:

Medical and Physical Care Plan for Campers with Medical Intervention Plans

(for use of Epipen or Albuterol, etc.)

For Any Camper Who May Require Prescription or Non-Prescription Medication
